Gage East Apartments is a permanent supportive housing complex that is in need of a full-time social worker that ensures housing stability for children and families.

You can make a difference by working with families to develop and meet goals that lead to improved skills in the areas of education, income, and well-being. The ideal candidate will possess the ability to work effectively with diverse populations and within a team environment.

Responsibilities (included but not limited to):
  • Provide direct service, guidance and support to families to facilitate access to needed resources in accordance with the family-centered case plan. This may include, but is not limited to: transportation, assistance with forms and applications to obtain services, and development and/or administration of activities to enhance daily living and life skills.
  • Maintain appropriate documentation and client records in accordance with federal, state and Center City Housing Corporation requirements. Complete timely data collection and provide appropriate written reports as assigned by the Program Supervisor.
  • Engage in outreach activities to assist with building and maintaining a network of resources while advocating for families.
  • Work with other case managers and staff to facilitate groups, organize and co-facilitate community meetings and coordinate other educational and community-building activities.
  • Collaboration with service providers and stakeholders is an integral part of this position as an avenue to address community needs.

  • Requirements:
    • MN Social Work license or license eligibility and/or bachelor's degree in a human services field with relevant experience working with families.

    Knowledge, Skills and Abilities:
    • Possess the ability to work effectively with diverse populations; to work within a team environment; effectively communicate with colleagues, co-workers and clients. Must possess computer literacy.
    • Self-motivation and the ability to work independently including prioritizing job duties.
    • Understand and be sensitive to the effects of trauma on an individual and its impact on successful residency, including an understanding of homeless populations.

    Sepcial Requirements:
    • Valid driver's license and access to vehicle for work purposes
