Job Summary

This position exists to support the Moving Home Minnesota (MHM) pilot project that coordinates access to cash assistance and health care programs for people leaving institutional settings to facilitate successful community-based supports and living. Cash assistance programs are: Minnesota Family Investment (MFIP), Supplemental Nutritional Assistance Program (SNAP), General Assistance, (GA), Housing Support (HS and formerly known as Group Residential Housing or GRH), and Minnesota Supplemental Aid (MSA).

The goal of the pilot program is to annually assist up to 200 adults within 30 - 60 days of relocation from institutional facilities into the community. The incumbent will:
  • Assist in identifying eligible persons and programs as defined by the pilot protocol.
  • Process combined application forms for people pending relocation from institutional facilities.
  • Work with facility case managers, program recipients, providers, counties/tribes to complete the application process and all programs started before relocation.
  • Coordinate with Moving Home Minnesota and the Housing and Support Services Division staff training for pilot project, training could be for counties/tribes, state staff, and providers.
  • Collect data and track outcomes and work closely with program evaluators.
  • Evaluate standards and determine best practices for pilot program and for future program work.
  • Settle disputes on county of financial responsibility and any other barriers that may prevent successful relocation from institutional facilities. Make policy recommendations to ensure better cash and healthcare program access.
  • Technical assistance to county staff, reservations, vendors/service providers, and advocacy groups.

The incumbent will also provide back-up support to the Housing and Support Services Division within the Minnesota Department of Human Services (DHS), and develop recommendations on supportive housing policy as it relates to persons with disabilities, people leaving correctional facilities and persons at risk of homelessness.

This pilot project is funded by MHM, federally known as Money Follows the Person, a rebalancing demonstration grant. This position is a critical component to the success of the individual's transition to the community, as well as the success of the MHM and MHM Tribal Initiative (TI) projects. The MHM project is currently funded through 2027, with the possibility of continued funding or to become a permanent program.

Qualifications

Minimum Qualifications

3 years professional working experience determining and providing eligibility for public assistance programs.

Experience must include:
  • Understanding of state and federal public assistance programs eligibility criteria and application process
  • Practical knowledge providing support for counties administer public assistance programs services
  • Familiarity with Home and Community-Based Services for service options availability and payment mechanisms
  • Use of MS Office Suite and programs such as: PowerPoint, SharePoint, video conferencing, MAXIS, and MMIS

Preferred Qualifications
  • Variety of experiences working effectively with others from different backgrounds and cultures
  • Strong human relations and communication skills; communicate clearly and professionally both verbally and in writing; respond openly to requests and concerns
  • Ability to give presentations and facilitate discussions to guide, solicit and explore diverse perspectives
  • Understanding of Medicare and Medicaid along with Social Security programs related public assistance

This position requires successful completion of the following:
  • Minnesota Department of Human Services (DHS) criminal background check will be conducted on all finalists for this position. Finalists must pass the DHS background check.
